A clinically competent, registered, professional nurse who prescribes, delegates and coordinates nursing care to the patient enrolled in Cardiac & Pulmonary Rehabilitation through the nursing process of assessment, planning, intervention, implementation, and evaluation. Effectively interacts with patient, significant others, and other health team members, while maintaining ANA standards of professional nursing practice. The CSC-Cardiac & Pulmonary Rehabilitation functions in a leadership role, overseeing daily unit operations, staff, and care delivery, utilizing a staff focused leadership model. Coordinates the care of patients enrolled in Cardiac & Pulmonary Rehabilitation, providing initial assessment & orientation. Develops rehabilitation plan that addresses the patient's risk stratification, risk factors, and resources. Communicates rehabilitation & discharge to ordering physician(s). Facilitates unit operations on a shift basis according to policy, procedure, and regulatory requirements. Coordinates unit based staffing assignments on a shift basis to achieve high quality and cost effective care. Directs patient care provided by staff nurses and support personnel for a specific shift. Champions new initiatives and helps in unit/departmental or hospital wide committees/task forces. Communicates directly with patients and significant others about nursing care. Implements appropriate action plans to provide safe, quality, and efficient care. Engages in appropriate hand-off (incoming and outgoing). Assures safety and security of patients, significant others, and staff (i.e. leads area safety huddles, ensures an environment of excellence). Assumes accountability for efficient and effective use of time and effort of all staff (i.e. ensures timely departures from shift). Provides direct patient care. In collaboration with the unit manager, contributes to the development of process improvement opportunities and initiatives. Monitors indicators for quality of care, patient safety and patient satisfaction. Assures regulatory compliance of all activities. Conducts evidence based research to identify performance models and benchmarks (internal and external) for data tracking and comparison. Assists with data interpretations and analysis. Clinical: Demonstrates clinical competence. Implements appropriate actions to respond to needs of patients and their significant others. Monitors and documents the patient's response to therapeutic interventions and progress toward outcomes. Demonstrates patient first and cost effective approach to patient care in terms of equipment, staff, supplies and all other resources. Assures that quality improvement activities are completed for the assigned shift. Assesses and intervenes in critical situations involving patients, staff, and visitors and consults with manager or administrator for guidance as needed. Professional: Exemplifies a professional image in appearance, manner, and presentation. Is an advocate for patients and their significant others, coworkers, the community, and the nursing profession. Ensures that the environment is clean, attractive, safe and free from hazards for patients, staff and visitors in accordance with regulatory requirements. Analyzes patterns of daily activities and recommends operational/program changes as needed. Anticipates staffing needs for subsequent shifts and makes necessary arrangements for coverage. Assumes responsibility for efficient and effective use of time and effort of all staff. Facilitates effective interdepartmental communication of incidents with appropriate personnel (manager, administrator, medical staff, etc.). Promotes philosophy and annual implementation plan objectives of Northwestern Memorial Hospital. Provides a milieu conducive to communication and cooperation among patients, staff and visitors. Identifies and communicates key issues and trends to managers and makes recommendations for improvement on an ongoing basis. Provides input to manager on budgetary issues, particularly staffing and cost containment strategies. Establishes relationships across departments to optimize patient care. Maintains an environment of excellence in his/her designated area. Provides coaching and mentorship to staff within his/her designated area. Participates in meetings, in-services, training, hospital wide committees/task forces, etc as designated per manager. Participates in interviewing and on boarding process. Education: Assesses and communicates staff development needs to manager. Orients staff to dept specific roles. Serves as a resource for staff. Contributes to unit orientation. Research: Assures that research activities are completed as necessary. Participates in nursing research activities. Participates in a Shared Decision Making (SDM) Council. AA/EOE.
Required: Registered Nursing license issued by the State of Illinois. Bachelors of Science Nursing. BLS through American Heart Association. ACLS through American Heart Association. Minimum two years clinical experience in area of clinical specialty. Prior healthcare leadership experience. Preferred: Additional clinical experience in another clinical area. Specialty certification in the clinical area of focus or obtain within 1 year of hire.
