Job Description

The principal role of the Outcomes Manager in Patient Care Services is to assist the patient care team in achieving the highest quality and best possible outcome for the patient in a cost-effective manner. The Outcomes Manager collaborates with members of the health care team across the continuum. Develops approaches to optimize the equity, effectiveness and efficiency of the delivery of resources available to the population both within and beyond the acute care setting.
Responsibilities:
- Outcome Monitoring & Analysis
- Collect, analyze, and interpret clinical outcomes data, including quality metrics, patient safety indicators, and regulatory measures.
- Benchmark performance against national standards and peer institutions.
- Identify gaps in care, emerging trends, and opportunities for improvement.
- Program Development & Implementation
- Design and implement evidence-based strategies to improve clinical outcomes and reduce variability in care delivery.
- Partner with clinical leaders, physicians, and frontline staff to embed best practices into workflows.
- Support the development of performance improvement projects using Lean, Six Sigma, or PDSA methodologies.
- Regulatory & Accreditation Compliance
- Ensure compliance with CMS, Joint Commission, and other accrediting body requirements related to quality and outcomes.
- Prepare reports and data submissions for external agencies and pay-for-performance programs.
- Education & Training
- Educate staff and leaders on outcome measures, performance dashboards, and quality improvement strategies.
- Serve as a resource for evidence-based practice and clinical effectiveness.
- Collaboration & Leadership
- Facilitate interdisciplinary meetings focused on outcome improvement.
- Engage stakeholders in setting priorities and aligning projects with organizational strategy.
- Act as a change agent to promote a culture of quality and patient-centered care.
Qualifications
Required
- Bachelors of Science in Nursing
- RN License
- Two years’ experience with the study of patient populations and working with care providers
- CPR – Basic Life Support
- Proficient with MS Office Products, primarily Excel and PowerPoint
Preferred
- Previous leadership experience
- Masters in nursing or related field
- CPHQ
