Responsibilities:
- This individual is responsible for overseeing the service experience for all transgender and gender diverse patients and for ensuring optimal use of all practice resources. The Gender Pathways Social Worker serves as a resource to patients and staff alike by establishing strong relationships, providing them with necessary information and advocating on their behalf as needed.
- Will assist in providing psychosocial counseling and intervention services to patients who request their services, or any patient referred by a provider.
- This individual will have the opportunity to work closely with Gender Pathways Program (GPP) leadership to advance the mission of expanding gender-affirming services, research, and advocacy at Northwestern Medicine.
- Answers incoming inquiries from patients, providers, staff and others related to all aspects of Gender Pathways Program operations.
- Assesses the needs of patients and schedules or coordinates the scheduling of appointments with GPP providers.
- Facilitates timely access to care and coordinates complex care processes throughout the Northwestern Medicine system.
- Facilitates the resolution/prevention of billing errors and denials – including obtaining pre-certifications, referrals, and appeals as needed.
- Conducts follow-up with patients to ensure understanding and satisfaction with plan of care.
- Evaluates patients’ physical and emotional recovery from procedures and strives to promote patient wellness and comfort.
- Provides patient education and/or resources as needed.
- Maintains a strong relationship with patients, community, internal providers and staff, external physician offices and vendors.
- Provides NM workforce education on transgender and gender diverse cultural competency.
- Assists GPP leadership with data collection and analysis related to GPP productivity, resource utilization, patient satisfaction, and clinical quality.
- May perform other duties as assigned.

The Social Worker, LCSW is responsible for providing psychosocial counseling and intervention services to patients who request their services, or any patient referred by a provider. The social worker advocates for the patient and family and assists in outpatient continuity of care.
Responsibilities:
- Completes a thorough psychosocial assessment for new patients.
- Utilizes critical thinking to identify and screen high risk social cases.
- Provides pre-treatment planning, psychosocial counseling/interventions, health education, financial counseling, referrals, and discharge/transition planning.
- Collaborates with team, other ancillary departments, case management staff, and social service agency providers to develop a care plan.
- Provides resources, including education materials, to patients and families in the areas of financial assistance, transportation, scheduling, and community resources to support the care plan.
- Supports department initiatives (attends case conferences, participates in quality improvement).
- May perform other duties as assigned.
Qualifications
Required:
- Master’s degree.
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the state of Illinois.
- Zero (0) to two (2) years’ experience in a relevant role.
