Job Description
The Case Therapist II reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
Responsibilities:
- The Primary Therapist functions as part of the treatment team by providing comprehensive care to people ages 18 and over with mental health and substance use disorders.
- Provides group and individual therapy for the treatment of mental health and substance use disorders.
- Develops and implements treatment plan in collaboration with patient. Assesses and reassesses patients psychosocial needs throughout program.
- Educates patients regarding their diagnosis, relapse prevention plan, treatment, and community resources.
- Coordinates patient care with relevant individuals, healthcare providers, and agencies in the community.
- Develops appropriate aftercare plans with in collaboration with patient.
- Participates in team meetings and helps in ongoing program development.
- Utilizes knowledge of the principles of growth and development over the life-span in assessing the patients status and age-specific needs.
- Adheres to and is responsible for the safety standards of Northwestern Medicine including the completion of the mandatory organizational safety standards on an annual basis.
- This position actively maintains confidentiality for our patients and their families as outlined in the Northwestern Confidentiality Policy and shows the same level of respect for every Associate with in Northwestern Medicine.
- This position performs other duties as assigned.
Qualifications
Required:
- Bachelor's degree in social work, psychology or related field
- LPC, CADC, LCPC, LCSW or LMFT
Preferred:
- Master's degree
- One year experience in individual and group therapy
