Easterseals PORT Health (ESPH) is seeking a dedicated and highly organized IPS Referral and Intake Coordinator. This role is responsible for 1915i referral process.
Your Role In Our Mission
The IPS Referral and Intake Coordinator functions primarily as responsible for the management and oversight of the 1915i referral process including coordination with Tailored Care Management (TCM) providers, ensuring 1915i assessments are completed and insurance is updated in MCO systems and NC tracks. This position may also assist with transitioning referrals from Standard Plan to Tailored Plan Medicaid. The IPS Referral and Intake Coordinator will work collaboratively with all IPS programs to ensure timely linkages to services.
Essential Job Functions
- Initial point of contact for Tailored Care Managers from MCOs and provider agencies.
- Review referrals, verify insurance in NC tracks, Provider Direct and ACS.
- Maintains a list of assigned TCMs and insurances, updates this list monthly and communicates with IPS Team Leads and Program Assistants.
- Schedule appointments with TCM for assessments.
- Communicate with IPS Teams regarding scheduled assessments, completion of assessments and PCPs.
- Assists with completing requests to switch insurance from Standard Plan to Tailored Plan Medicaid
- Ensure that copies of the PCP and assessments are uploaded into Welligent in a timely manner
- Ensure the accuracy of payer sources in Welligent
- Follow-up on treatment recommendations to ensure services have been initiated.
- Provide and maintain community resource information for members and staff.
- Serves as a liaison between individuals, families, Easterseals PORT Health employees, MCOs, and TCM providers.
- Completes all paperwork in a timely and accurate manner.
- Provides high quality customer service, and is a positive representative for the agency.
- Other duties as assigned.
- Follows all ESPH procedures as appropriate to position.

Minimum Qualifications
What We’re Looking For :
- Bachelor’s degree from an approved accredited institution; human services preferred.
- Experience working with MCOs, VR, Medicaid
- Knowledge of DSM-V diagnoses, provider service options and community resources required.
- Demonstrates high moral and ethical character and be free of any substantiated charges of abuse or neglect on the NC Healthcare Registry and OIG Office of Inspector General.
- Possess valid, unrestricted, state-appropriate driver’s license and unrestricted, valid insurance for person vehicles utilized for work.
