Job Description
The Intake Coordinator HME reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
- Under the direction of the Director of Home Medical Equipment the Intake Coordinator is responsible for receiving referrals requesting the level of service in the continuum of care. Review the patients’ chart to obtain demographic, insurance and relevant clinical information. Verify the orders and health insurance benefits meet the qualification requirement to receive Home Medical Equipment according to Medicare, Medicaid and third party payer guidelines.
Responsibilities:
- The Intake Coordinator is responsible for gathering information of new referrals, identify Home Medical Equipment needs of patients, obtain orders from the patient’s physician, and to begin the smooth transition from the hospital environment to the home setting.
- The intake coordinator has direct contact with Physicians, Nurses, and Discharge planners to coordinate patient needs and the delivery of Home Medical Equipment in a timely matter.
- The Intake Coordinator negotiates contracts/payments with third party vendors not contracted with CHS if the patient meets criteria for Home Medical Equipment services.
- Other duties as assigned by Home Medical Equipment Manager.
Qualifications
Required
- High School diploma or equivalent
- Strong communication skills in speaking and writing
- Computer experience
Preferred
- Associates Degree or higher
- Experience in Home Medical Equipment
