Behavioral Health Care Manager to join our collaborative, multidisciplinary Tailored Care Management (TCM) team in
Lenoir and Duplin counties.
What You’ll Do
PORT Health’s Tailored Care Management (TCM) team provides whole-person, integrated health care through evaluation, assessment, care planning, coordination, tracking, and monitoring outcomes of individuals served. The Care Manager focuses on closing gaps for physical and mental health, social determinants of health, and other non-health-related needs.
This position travels within Lenoir and Duplin counties.
How You’ll Benefit
You’ll earn a
competitive salary between $51,000 and $53,000, depending on skills and experience. This full-time position offers a
flexible schedule Monday-Friday from 8 am-5 pm. We also offer competitive benefits for eligible positions.

What We’re Looking For
To join our team, you must be willing to provide services in various community locations as needed, rotate after-hours support coverage, be knowledgeable in Microsoft Office Suite, have experience with Electronic Health Records software, have a passion for helping others, and be able to link clients to community resources. We also require the following:
- A Bachelor’s degree in health, psychology, sociology, social work, nursing, or another relevant human services area; a Master’s preferred.
- Able to be credentialed as a Mental Health Qualified Professional.
- Minimum of 2 years of experience with the mental health and/or substance use disorder population.
- A valid driver's license, current auto insurance, and a good driving record.
- Must be able to work independently, have strong verbal and written communication skills.
