Your Role in Our Mission
The
Referral and Entry Coordinator plays a key role in evaluating the eligibility of individuals served by the REACH program. In this dynamic position, you’ll review and process referrals, maintain accurate records, and share program information with community partners. You’ll build meaningful connections with individuals, families, and local systems to ensure a smooth start to services. You’ll also manage spreadsheets, generate reports, and track program data. No two days are ever the same! Additionally, you’ll occasionally support our Mobile Crisis Team as part of the on-call rotation, making a direct impact when it matters most.
This position is primarily remote but may require occasional travel to client homes for intakes or during on‑call shifts.

Compensation & Benefits
- Competitive salary: Starting from $65,000 - $70,000 for this full-time exempt position
- Sign-on & retention bonus is paid out upon successful completion on 90 days, 6 months, and 1 year!
- Consistent, hybrid Monday-Friday 8am-5pm schedule, with occasional on-call shifts

What We’re Looking For
- Education: Master’s in health, psychology or social work
- Licensure: Licensed or Licensed-Eligible as a LCSW, LPC, LMFT
- Skills: Crisis intervention, adaptability, problem-solving and decision making
- Experience: Working with the I/DD population, including programming for individuals with significant behavioral needs.
- Maintain a valid driver's license, good driving record and current auto insurance.
- Proficiency with EHR systems, Microsoft Suite, and technically savvy.
