Join Easterseals PORT Health (ESPH) as a Family Partner on our Wraparound Services team. In this impactful role, you’ll use your own experience to empower families, guide them through complex systems, and support them in becoming strong advocates for their children. As an essential member of the Child and Family Team (CFT), you’ll help shape care that is family-driven, culturally responsive, and grounded in hope and resilience.

What You’ll Do
The Family Partner uses lived experience raising a child with behavioral health needs to provide one-on-one, strengths-based support to families. As a key member of the Child and Family Team (CFT), the Family Partner empowers caregivers to advocate for themselves, navigate services, and build resiliency. This role involves mentoring families, supporting care coordination, connecting them to community resources, and promoting positive outcomes through collaborative, family-driven planning.

What We’re Looking For
To join our team, you must be willing to provide family-centered support in home and community-based settings, collaborate with care teams, assist families in navigating systems and accessing resources, and participate in crisis phone support rotation. We also require the following:
- Must be a parent or primary caregiver of a child who has received services
- Minimum of a Bachelor's degree and 2-year's of experience working with youths and navigating child and family serving systems
- A valid driver's license, current auto insurance and a good driving record
- Experience promoting client “voice and choice” preferred
