Admission / Discharge Coordinator to join our VA REACH team, supporting our homes in Culpeper and Woodbridge, VA.

Your Role in Our Mission
As the Admission / Discharge Coordinator, you will provide admission and discharge support to our Youth Crisis Therapeutic Home, Adult Crisis Therapeutic Home, and Adult Transitional Home. You will participate in the Admission Advisory Committee for our Adult Transitional Home and serve as a liaison with community stakeholders. You will be responsible for all discharge planning - including support team meetings, community appointments with the goal of transition, and overall discharge communication. In addition, this position maintains individual support plans and participates in the residential on-call rotation.

This hybrid position follows a Monday - Friday 9am-5pm schedule, with additional flexibility needed for the rotating residential on-call schedule. Additional stipends are provided for when you are working on-call!

Compensation & Benefits
- Competitive salary: $60,000 to $65,000 annually| $28.85 - $31.25 hourly, base for this full-time position
- Sign-on and retention bonus will be paid out to FT, new hires upon successful completion of 90 days, 6 months, and 1 year!

What We’re Looking For
- Education: Bachelor's degree in a human service or related field
- Qualified Mental Health Professional, or eligible to obtain QMHP-T within 30 days of start
- At least 1 year of experience with both both developmental disabilities and/or mental health populations
- Crisis prevention and intervention experience preferred
- Maintain a valid driver's license and auto insurance
