Intensive In-Home (IIH) Qualified Professional

Easterseals Port Health (ESPH) has an immediate need for a full-time Intensive In-Home Qualified Professional. Join this team of smart, talented professionals working every day to improve the lives of people and families. You will flourish in this role if you’re self-motivated, require minimum supervision, and is a go-getter!
Responsibilities Include And Are Not Limited To
- Provides crisis, case management, and individual/family therapy to prevent the need for out of home placement for children diagnosed with a mental health disorder
- Ability to deliver essential services in various environments such as:
- Schools
- Homes
- Detention Centers and Jails (State funding only)
- Homeless Shelters
- Street Locations
- Responsible for coordinating initial assessment and the development of the youth’s Person-Centered Plan (PCP)
- Ability to evaluate the nature of the crises, intervene and diffuse the crises likelihood of a recurrence
- Ability to link needed community services and resources to clients
- Provide parenting skills training to aid the family build fundamental skills needed for coping with the youth’s disorder
- Work with caregivers in the implementation of home-based behavioral supports
- When assigned, provide “First Responder” crisis response on a 24-7, 365 basis to recipients
Requirements
- Must have either your:
- Bachelors degree in the Human Service field with two years post-degree experience with the at-risk youth population, or
- Masters degree in the Human Service field with one-year post-degree experience with the at-risk youth population
- Must have a valid, unrestricted, state appropriate driver’s license
- Must possess the visual acuity to develop written correspondence and determine accuracy, neatness, and thoroughness of the work assigned.
- Must be able to physically perform the basic life operational functions of talking, hearing, sitting for long periods, and performing repetitive motions, such as using a computer.
- Must be able to speak audibly, write legibly and perform repetitive motions such as typing.
