Service Coordinator in our Empower programs, you’ll play a key role in helping individuals with developmental disabilities live their best lives. From guiding Recreation & Wellness and Transition programs to ensuring services meet the highest standards, you’ll be the connection between quality care, personal goals, and community resources. Job Title: Service Coordinator – Empower Program
Location: Greenville, NC on-site
Department: Empower – Recreation & Wellness / Transition Programs
Employment Type: Full-Time, Exempt
Work Schedule: Monday–Friday, 8:30 AM – 5:30 PM
Key Responsibilities
- Manage participant enrollment, ensuring all required consents, authorizations, and documentation are complete before services begin.
- Support the development and implementation of person-centered plans and short-term goals; monitor and report progress quarterly.
- Collaborate with Managed Care Organizations (MCOs), internal teams, and community partners to ensure coordinated, needs-based supports.
- Review and approve service documentation to ensure it meets Medicaid, Innovations Waiver, and other NC funding standards.
- Conduct and submit required Clinical Monitoring for funded services.
- Maintain accurate and current participant records in the electronic health record (EHR) system.
- Attend care coordination meetings and provide updates on participant progress.
- Assist with coverage of other service areas, such as Recreation Paraprofessional or Job Coach, when needed.
- Ensure all services uphold participants’ rights, dignity, and safety.
- Complete required training (NC-SNAP, Relias) and engage in ongoing professional development.

What We’re Looking For
- Bachelor's degree from a college or university accredited by an entity approved by USDOE or CHEA.
- Minimum of Two (2) years’ experience working with individuals with Intellectual and / or Developmental Disabilities, and meets qualifications for Qualified Professional.
- Must have a valid, unrestricted, state appropriate driver’s license and personal vehicle insurance.
- Satisfactory criminal background check and acceptable motor vehicle record.
- Knowledge of programs such as Microsoft Word, Microsoft Excel, Zoom and Microsoft Teams.
- Experience with EHR a plus.
