VA REACH team in Charlottesville, VA and the surrounding areas.

Your Role in Our Mission
As the
Hospital Liaison, you will make a meaningful impact in the lives of those experiencing crisis. You will provide support to REACH clients who are hospitalized at state or local hospitals within the DBHDS. This position serves as the primary contact for discharge planning and facilitates admission to the REACH Crisis Therapeutic Home or REACH Mobile Crisis Supports, if appropriate. This position will also serve as the primary Coordinator for REACH services, which may include the completion of a comprehensive intake assessment, until the case is fully opened and assigned for on-going REACH services. This role requires travel to state and local hospitals and participates in our rotating on-call schedule.

Enjoy a consistent, Monday-Friday 9am-5pm schedule, with additional availability needed for our on-call rotation.

Compensation & Benefits
- Competitive salary: $60,000 - 63,000 annually

What We’re Looking For
- Education: Minimum of a bachelor’s degree in human services or related field
- Must obtain, or be eligible to obtain, QMHP-T registration with the Virginia Board of Counseling within 30 days of start.
- Licensed or license-eligible as an LPC, LCSW, or LMFT preferred
- Skills: Crisis intervention, adaptability, problem-solving and decision making.
- 3 years’ experience with mental health populations and I/DD preferred
- Knowledge of Virginia Hospital & Community Based services required
- Maintain a valid driver's license, good driving record and current auto insurance.
- Proficiency with EHR systems, Microsoft Suite, and technically savvy.
